Dividend payment tax statement

We are legally required to provide a tax statement with your social insurance number (SIN) or your Federal Business Number for dividends over $100. If this applies to you, we invite you to fill out the form below.

Please note that your tax receipt will be mailed to you in February 2027 and that income tax has already been deducted from your dividend amount.

Important: Promutuel Insurance will never request your SIN or Federal Business Number by email or phone. This information must be submitted exclusively via the secure form below.
