Dividend payment tax statement

We are legally required to provide a tax statement with your social insurance number (SIN) or your Federal Business Number for dividends over $100. If this applies to you, we thank you for filling out the form below.

Note: your tax receipt will be mailed to you in February 2025 and tax has already been deducted from your dividend amount.

By submitting this information, you consent to it being collected for the issuance of your 2025 member dividend. It will be used by Promutuel Insurance for tax report or slip purposes only, as required by tax authorities, and it will be destroyed within the statutory retention period.
